Agreed terms for Pain Management Program

Services
As part of providing health services to you, your therapists and doctor will need to collect and record personal information as part of the assessment, diagnosis and treatment process. The information is only viewed by your therapists and doctor and is retained securely. If there is information you do not want recorded, please inform your health professional.
Empower Pain Program
As part of your Empower Pain Management program you may see a physiotherapist, psychologist, and a pain specialist (medical doctor).
Sessions and Fees- Psychology or Physiotherapy
Each session is booked to allow for therapy as well as administration time. The costs per session per therapist are as follows:
50 minute psychology or physiotherapy consult: $225 per therapist per session ($175 for concession card holders) unless the psychologist is clinical endorsed whereby the cost per session is $250/($200 for concession card holders)
25 minute consultation: $115 per therapist per session ($90 for concession card holders).
Fees are as of 01/07/2025.
Please confirm any other funding for your sessions prior to commencing treatment.
The total cost for each session is payable prior to the start of the session by cash or EFTPOS. It is your responsibility to attend sessions at the time scheduled. It is your responsibility to ensure eligibility with the relevant fundings bodies (e.g., Medicare, TAC or Workcover), and you have responsibility to pay any invoices if not eligible within these bodies.

Initial Assessment 1 hour: $390 ($370 concession), Medicare rebate $142.60
Standard review 20 minutes: $210 ($185 concession), Medicare rebate $71.40
Extended review 40 minutes: $260 ($210 concession ), Medicare rebate $71.40
Network Pain Management Program under WorkCover or TAC All consultation fees are billed directly to Workcover or TAC
WorkCover or TAC Pain Specialist Consultation:
Initial Assessment- $390 rebate $268 Payment is required at time of consultation and seek own reimbursement.
Review – billed directly to TAC/WC or $165. Review consultations will be billed directly to TAC/WorkCover Insurer only if we have received written acceptance of the consultation from the insurer. It is your responsibility to ensure this has been provided at the time of billing, otherwise payment by you is required at time of consultation.
Payment if applicable is required at that time of consultation.
Access to Client Information
At any stage you as a client are entitled to access to the information about you kept on file, unless the relevant legislation provides otherwise. The therapists may discuss with you appropriate forms of access.
Reminders
Empower Rehab & Psychology will endeavour to provide SMS reminders for your sessions if you have provided a mobile number. This is a courtesy service and does not influence the cancellation policy.
Cancellation Policy
If you need to cancel or postpone your appointment, please provide at least 48 hours notice by contacting reception via phone 9459 3344 or email reception@empowerrehab.com otherwise you will be charged a cancellation fee for physiotherapy and psychology sessions of $75 for 50 minute ($40 for 25 minute) sessions; and $100 for medical sessions. For all session types the full fee is payable if you fail to attend (DNA) without notice. Cancellation and DNA fees cannot be claimed through Medicare, health insurance or compensable bodies, such as Workcover and TAC. If reception is not available, please leave a voicemail.
Confidentiality
All personal information gathered by the therapists during treatment will remain confidential and secure except where:
- It is subpoenaed by a court, or
- Failure to disclose the information would place you or another person at serious and imminent risk; or
- There is reasonable suspicion of abuse to a child or if disclosure is otherwise required or authorised by law; or
- Your prior approval has been obtained to
- provide a written report or information to another professional or E.g., a GP or a lawyer; or
- discuss the material with another person, g., a parent or employer.
Your therapists engage in ongoing professional supervision to ensure the best quality service. Within these sessions details of client’s situations may be discussed. This is done within a confidential professional relationship. All session content is confidential. Please do not record sessions, unless you have permission.
In any instance where multiple attempts are made to be in contact with you regarding appointments or payments, and we cannot establish contact with you, we will contact your listed next of kin to ensure your wellbeing. We will leave a message to inform you that we will be doing so.
Provision of a Telehealth Service

Empower Rehab can provide Telehealth services in certain situations. To support the security of your personal information this practice uses Telehealth software. The privacy of any form of communication via the Internet is limited by the security of the technology used. Signing below indicates consent to attend Telehealth treatment, acknowledging limitations to privacy and security. A Telehealth consultation is subject to limitations such as unstable network connection, which may affect the quality of the session.
Privacy Agreement and Patient Consent:
Empower Rehab & Psychology and Dr Eddy complies with the Privacy Act (1988) and is committed to protecting the privacy of individuals and their personal information. We use email (privacy measures are in place but possible security risks remain) and eFax to communicate with you and other relevant parties in order to avoid delays that occur with postage.
My signature below indicates that I have read and agree to all the above; and consent to Empower Rehab and Dr Eddy collecting, using and storing my personal information; the release of relevant personal information to other health professionals to allow quality care; and in the case of a WorkSafe, TAC or other relevant third party related consultations or service, the release of relevant clinical information to my employer, their authorised representative and/or insurer.
I understand I may withdraw my consent for Empower Rehab and Dr Eddy to use and disclose my personal information at any time (except when legal obligations must be met by Empower Rehab and Dr Eddy).
Crisis Support
Please be aware that Empower Rehab & Psychology do not have a crisis service. However, the following services are available 24/7: Life Line: 13 11 14 North Eastern CAT Service: 1300 859 789. In the event of a crisis requiring immediate intervention, call 000 or attend the emergency department of your local hospital.
